DBC Race Team Clothing

NOTE:

• We are now accepting requests for the second (and final) clothing order.
• Completed order forms (see below) are due by Friday, 4 April 2008.
• DBC racers need to include a (minimum) 50% deposit check (payable to DBC)
• All others (including general DBC members), please include entire payment (we will not deposit your check until after you receive the order).

How to order Race Team clothing:

1. Save (download) this Official DBC Race Team Clothing Order Form (RTF format -- should work with most word processors). If you don't have a word processor handy, you may print the PDF version and fill it out by hand.
2. Fill in all the appropriate items on the form with your word processor.
3. Send the order form file, as an e-mail attachment, to Shawn Miller.
4. Send a check (50% deposit for DBC racers; 100% for all others) to Shawn via US Mail (Shawn's address is on the form).
5. Patiently wait for the order to arrive in late May.
